The UO Ticket Office is an authorized U.S. Government Passport Acceptance Facility. We’re here to make your passport experience as convenient and easy as possible. No appointments, just drop by. We have all the necessary forms and we can take your photo on-site. We also offer expedited service.
Passport services are walk-up only and are provided on a first-come, first-served basis. Please sign in on the iPad when you arrive to enter the processing queue.

Applicants 16yrs & Older - applying in person at an Acceptance Facility
- DS-11 form completed in black ink only
- Proof of U.S. Citizenship
- Previously issued U.S. passport, certificate of naturalization, or original birth certificate
- Government-issued photo ID
- Please note: if you are applying with an out-of-state ID, you will need a secondary piece of ID
- Any other necessary supporting documents, if applicable
- Check or money order for passport fees
- Please note: Acceptance fees ($35) and Photo fees (if purchasing on-site) are separate from passport fees and can be paid with card or cash. All other passport fees must be check or money order paid to U.S. Department of State at the time of application. See below for more details.
Applicants Under 16 Years of Age - must apply in person at an Acceptance Facility
- DS-11 form completed in black ink only
- Proof of U.S. Citizenship

- Minor’s birth certificate
- Applicants 15 years or younger must apply in person with all parents or legal guardians listed on the minor's birth certificate at the time the application is submitted.
- If one or more parents or guardians cannot apply with the minor child, more supporting documents will need to be submitted.
- Government-issued photo ID for all parents or legal guardians
- Please note: if you are applying with an out-of-state ID, you will need a secondary piece of ID
- Any other necessary supporting documents to apply for a minor child, if applicable

Please note that processing times begin the day an application is received by the Department of State at a Passport Processing Agency, not the day applications are submitted to our Acceptance Facility.
Payment for passport books and/or cards must be in the form of a personal check, cashier’s check, or money order made out to the U.S. Department of State.

3. Acceptance Fees
Do not combine Passport fees and Acceptance fees. Passport fees must be paid by check or money order to the U.S. Department of State as detailed above. Acceptance fees are the same anywhere you choose to apply and are paid separately to the UO Ticket Office / Acceptance Facility and can be paid by card or cash at the time of application.

Where to Find Us and Where to Park
On-Campus Parking: There are sections of pay-to-park spaces along University Street near East 18th Avenue as well as East 13th Avenue near Agate Street. Please heed all UO parking signs and rules. CLICK HERE for additional information on UO campus parking.
Off-Campus Parking: Eugene city-metered parking spaces are available in many areas around the UO campus perimeter, including: along East 18th Avenue on the south edge of campus; along Agate Street on the east edge of campus; and along Kincaid Street on the west edge of campus. All of these are a brief walking distance to our office in the Erb Memorial Union (EMU).
